Case management incident follow-up inspection regarding unusual incident from 03/14/2024. Two children left unattended in outside play yard for less than 2 minutes while teacher opened door for parent pickup. Type B deficiency cited for violation of care and supervision requirements. Facility has requested Technical Support Program. No injuries sustained.

Case management follow-up confirmed that a child fell from a play structure on 2022-07-08 and sustained minor scrapes. Interviews showed staff were supervising children in groups, but the incident was still cited as a lack of supervision.

During a case management health check, LPAs observed 55 children in care with 8 staff. Two staff members were found without fingerprint clearance and were directed to leave immediately, creating an immediate health and safety citation.
