The incident follow-up found that a child was injured when the play-yard landscaping allowed a trip into a brick wall. Investigators linked the head injury to uneven terrain in the yard and issued a Type A citation.

The inspector followed up on a late-reported unusual incident from December 22, 2021 in which a child tripped over wooden playground logs and hit their head, causing a bump and bruise. Interviews indicated staff were nearby, there was no lack of supervision, and staff responded appropriately with first aid and parent notification, but the facility was cited for failing to report the incident on time.

LPAs conducted a case management inspection to follow up on a child injury incident from April 21, 2021 that had not been reported on time. They reviewed the event in which a child's finger was caught in a classroom door as a staff member exited, interviewed staff and a parent, and collected photos, injury images, and the facility roster. The visit resulted in a Type B citation for late reporting and a Type A citation because the incident posed an immediate health and safety risk.
