A COVID-19 risk assessment was conducted prior to entering the facility. LPA wore appropriate personal protective equipment. The purpose of this inspection is to follow up on an unusual incident report that occurred on 04/18/23. The facility reported this incident to the Department within the required 24 hours. Upon arrival LPA met with Assistant Director Angel Haili who guided LPA on a tour of the facility. Director Melissa Garcia later arrived at 9:00AM. Census was taken. On 04/18/23, an incident was self-reported to the Department via telephone by the facility who reported, staff alleges that a child's personal rights were violated by staff while in care.
