Complaint investigation substantiated allegations of inadequate supervision resulting in child injuries. Despite meeting staff-to-child ratios (2 teachers for 16 children), supervision gaps allowed injuries including biting and cabinet overturning incidents.

This case management follow-up confirmed a supervision lapse during an outdoor-to-indoor transition. A child had been left unattended on the playground for about one minute before staff found the child outside the classroom door.

This incident follow-up found evidence in video footage that a staff member abruptly removed a child from an activity on the playground, infringing the child's personal rights. The facility was instructed to provide the report and acknowledgment form to parents.
