What the inspector wrote

RP alleged, “Staff handled child in a rough manner” and “Staff yelled at child” Facility submitted an Unusual Incident Report (UIR) that was reported to the facility within 24 hours of the occurrence of the incident. The incident was to report the aggressive behavior of a parent at the facility towards staff. LPA, Tuba obtained from the Director behavior reports and staff notes of the behavior of C1. S2 did disclose that while they do not yell, they do have a loud voice and may come across loud as they are attempting to communicate over the children who are also being loud. S2 did disclose that C1 did hide under the table and there was an incident on March 21, 2025, with C1’s parent who was very upset. S2 disclosed that C1 had challenging behaviors with staff and other children. S1 and Assistant Director also agreed that C1 had challenging behaviors, however S1, S2 and Assistant Director stated that children who are having difficulties with behaviors, the method used is redirection. S1 stated that they use a behavior chart to encourage positive behaviors. P1, P2, P3, C2 and C3 were interviewed, but no disclosures were made. LPA, Tuba interviewed P4 who stated that their child had been grabbed and yelled at while in care and stated that they had several videos due to the facility having cameras in each classroom but it does not have any sound. LPAs, Tuba and Ortiz reviewed the videos and did not see any staff grab children in care at those times. LPA, Tuba advised as best practice to the Assistant Director that more staff training's and meetings is encouraged to assist staff to handle children with behavioral issues as well as Technical Support Program (TSP). Brochure was provided. Based on interviews with P4, and S2 the allegations, “Staff handled child in a rough manner” and “Staff yelled at child” is deemed UNSUBSTANTIATED. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur. A Notice of Site Visit was provided and must be posted for 30 days. An exit interview was conducted, and a copy of this report was provided to the Assistant Director, Susan Stubbs. page 2 of 2
