This was a complaint investigation that found a personal rights violation involving the use of a toddler chair for discipline or restraint. The inspector observed the child could sit independently in a regular chair, and the allegation was substantiated.

During this unannounced annual continuation visit, the analyst reviewed children, staff, and facility records and confirmed monthly fire and earthquake drills were being logged. The visit found one staff member missing measles vaccination documentation, while all present had background clearance or exemption and CPR/Pediatric First Aid coverage was in place.

A complaint involving an allergy-related food mistake was substantiated after the analyst reviewed the child's file. The file showed the children had different allergies, undermining the director's explanation for serving the food product.
