What the inspector wrote

On 02/12/2026, Licensing Program Analysts (LPAs) Mary Silva and Caroline Nguyen conducted an unannounced Case Management Inspection to ensure the facility is in compliance with the health and safety standards in accordance with California Code of Regulations Title 22. LPAs met with Director Brenda Garcia, to whom the reason for the visit was explained. LPAs were guided on a tour of the school age program. Census was taken and LPAs observed 3 school age children with 1 teacher in room #1, room #2 and room #3 were not occupied by children or staff. During the tour of the school age program, LPAs entered the gated play yard and observed debris in form of leaves. Per director, maintenance was previously conducted weekly and currently conducted monthly. LPAs observed the 6 black square shape tile padding in the outdoor playground and under the three play structures are not in safe condition; the black cushion padded materials are loose, ripped, overlapped and expose the concrete. LPAs observed the play structure with metal slide to have a crack at the end/bottom of the unit. LPAs observed two air conditioning (AC) units outside the windows of room #1 protruding into the play yard; the AC filter is covered with a metal mesh and is warm to the touch. Per director children have run into the air unit causing injuries. Pictures were taken of the school age play yard. Based on LPAs observations and photographs obtained, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Appeal rights provided. Exit interview conducted and report was reviewed with Director Brenda Garcia.

The play yard was observed to have leaves through out the area, which poses a potential risk of the health, safety, and personal rights of children in care.
