What the inspector wrote

Licensing Program Analysts (LPA) Cynthia Reyes conducted an unannounced case management inspection on this date. This inspection is to ensure the health and safety standards as required by the regulations, Title 22, child care centers are met for this School Age program. LPA met with Ana Lopez- Teacher, who the inspection was gone over and a tour of the facility was conducted. Director Brenda arrived back to the site as she was already off for the day. During the walk through of the facility on 03/11/2026 with Director Brenda Garcia and interviews and records received and reviewed the following citation was determined to be true. (Citation cited on this date due to lack of time on the other date listed). An incident occurred on 03/02/2026 where a child fell off a broken swing in the preschool play yard. During that investigation it was determined that the child is 4 years old and is enrolled in the school age program and due to her age should be enrolled in the preschool program, Even though she is enrolled in the school age she was still playing in the preschool yard as she should have been enrolled in the preschool. Also during interviews it was determined that the school age children commingle some times in the morning with the preschool children before they go to there regular school and most times they commingle with the preschool children at night before they go home. The facility has not requested a waiver for the children to commingle at the start or end of the day. On 03/11/2026 LPA conducted interview with a couple staff members however due to time restraints additional staff and children were not interviewed at that date and were conducted on this date. Based on the LPA observations, interviews conducted and records reviews, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health and safety. Page 2 A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the Director Brenda Garcia LPA, Cynthia Reyes informed Director Brenda Garcia, that this report dated 01/29/2026 document(s) (1) Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety or personal rights of a child in care. Also, LPA, Reyes informed Licensee, to provide a copy of this licensing report dated 02/23/2026 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC9224), or written statement, must be placed in the child’s file for verification.
