What the inspector wrote

Visit conducted in Spanish On 12/05/2025 Licensing Program Analyst LPA(s) Mary Silva and Diana Ortiz conducted an unannounced Case Management Inspection to the above facility. LPAs met with Licensee Elena Salazar, to whom the reason for the visit was explained. LPA(s) were guided on a tour of the facility. Census was taken. LPA(s) observed 3 children, 1 infant and 2 preschool children. Assistant #1 was present during the visit. During the visit, LPA(s) conducted interviews with licensee, assistant #1 and reviewed files for children and staff. Based on the information reviewed, and interviews conducted assistant #2 transports children, and assists with changing children’s clothes and routine activities. Therefore, it was determined that assistant #2 does not meet the qualification to provide care and supervision for children in care. Per file review assistant #2 has not completed mandated reporter training, First Aid and CPR certification, and does not have verification of immunization records( TDAP, MMR and Influenza). Based on the information obtained the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00 Exit interview conducted and report was reviewed with Licensee Elena Salazar.

    Employees or volunteers at family day care home; immunization requirements; records; exemptions: (a) (1) Commencing September 1, 2016, a person shall not be employed or volunteer at a family day care home if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year. Requirement was not met by.. Based on observations, interview and record review assistant #2 has not met this requirement. This poses a potential health and safety risk to the children in care.
