Inspection was conducted in Spanish On 02/03/26 at 8:40am, Licensing Program Analyst (LPA) Diana Ortiz conducted an unannounced annual inspection to the above facility. LPA met with Licensee, Elena Salazar, to whom the reason for the visit was explained. Census was taken and LPA observed two (2) children present at the time of inspection. Per Licensee, there are 14 children currently enrolled. A current children’s roster was available for review. Hours of operation are from 6am-6:30pm Monday through Friday. The licensee was observed to be operating within the license capacity limitations at the time of inspection. LIC 126 Entrance Checklist was provided to Licensee. LPA observed the following required posted documentation in the main entry way of the facility: Facility License, Publication (PUB) 394- Notification of Parent Rights and Licensing Form (LIC) 9148- Earthquake Preparedness form. LPA reviewed facility records for LIC 9040- Facility Roster, LIC 610- Facility Disaster Plan. This is a one-story home which consists of 2 bedrooms, 1 bathroom, kitchen, dining room, living room, enclosed patio, fenced front yard and fenced backyard. Currently residing in the home are two adults and no minor children. There was a total of one (1) adult present during this inspection, licensee. All adults present at time of inspection are fingerprint cleared and associated to facility in Guardian. Areas accessible to children were inspected as follows: Kitchen, living room (daycare room), dining room (daycare room), enclosed patio (activity area), one bathroom and front yard. Page 1 of 4 Areas off limits include: 2 bedrooms, and backyard. During this visit, all areas identified on the facility sketch that are accessible for children to use were toured and inspected. The Licensee uses the front yard for outdoor play. The front outdoor play areas were observed to be fenced. Licensing staff observed the outdoor yard to have toys and other materials for children to play with such as playhouses, tricycles, arts and crafts and trucks. There is adequate shade in the front yard. Licensee states that she is part of the Options for Learning program and provides the following meals: Breakfast, lunch, and PM Snack. Parents have a choice to bring in their child’s lunch if they choose to. LPA advised the Licensee that any food that is provided by parents must be labeled with child’s name with the date and stored appropriately. Per Licensee, there is central air and heating in the home. Per Licensee, the ill/isolation area is in the dining room/daycare room. There is an area in the room where the children will wait for parents or authorized representatives to pick them up. There are cots and mats available for ill children to rest if needed. Detergents, cleaning compounds, and medications were observed to be inaccessible to children. LPA advised that any poison should be locked under key or combination lock. LPA did not observe any pets at time of inspection and Licensee confirms she has no pets. Per Licensee, there are no weapons, firearms in the home and there are no bodies of water around the premises. LPA did not observe any weapons/firearms, or bodies of water around the premises at time of inspection. The valve on the required 2A 10BC fire extinguisher indicates fully charged. The fire extinguisher was last serviced on 02/10/2026 and service tag is attached. Smoke and carbon monoxide detectors were tested and were operable on this day. All homes shall conduct fire and disaster drills at least once every six months and document the date and time of each drill. Last drill documented was conducted on 01/13/2026. The home is observed to be clean and orderly. There are first aid supplies available near the entrance of the daycare area. LPA observed that there are age-appropriate toys available for children. Appropriate sleeping arrangements in form of cots and mats were observed. Page 2 of 4 Child’s records were reviewed for (LIC) 282- Affidavit Regarding Liability Insurance, Immunization's Records, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, and LIC 995A Notification of Parents’ Rights. Licensee file was reviewed for the following: LIC 508- Criminal Record Statement, Proof of immunizations against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement Acknowledging Requirement to Report Child Abuse. Pediatric First Aid and CPR certification expires 10/31/2027 for licensee, and for licensee assistant. Mandated Reporter Training has been completed and expires on 08/2026 for licensee and for assistant. Smoking is prohibited in a licensed Family Child Care Home. Per Licensee, no one smokes in the home. LPA did not observe the following items during the inspection: Johnny Jumpers, Saucer Chairs, and/or any other item that fall into these categories are not permitted in a family childcare facility. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Page 3 of 4 Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the Licensee Elena Salazar, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
