What the inspector wrote

On Monday, October 6, 2025, Licensing Program Analyst (LPA) Perry conducted a case management – other inspection at the above facility to follow up on forms and documentation requested by the Department. A letter was sent to the facility on October 2, 2025, notifying the licensee of missing documentation. The facility had previously requested to add Staff #1 and Staff #2 onto the facility roster; however, the Department was unable to process the transfer without the required forms and identification. Upon arrival, LPA Perry met with the Facility Director, Sister Irma Padill a who guided LPA Perry on a tour of the facility. LPA explained the purpose of the visit and confirmed that the Department is requesting the facility to complete and submit an LIC 9182 Criminal Background Clearance Transfer Request forms for Staff #1 and Staff #2 and legible, valid, government issued photo identification for both staff, to be submitted to the Department for processing. LPA reminded the Director that all adults working or residing in a licensed facility must have either a California background clearance or a criminal record exemption clearance associated to the facility prior to having any client contact. LPA informed director that failure to comply is a violation of law subject to an immediate $100 civil penalty per person, per day until the individual is cleared. Any individual without clearance must be immediately removed from the premises. LPA Perry confirmed per facility director that Staff #1 and Staff #2 are currently volunteers and are volunteering less than 15 hours a week and per director staff #1 once fingerprint cleared will be hired as a full time Teacher. Staff #2 is a volunteer and is pending fingerprints to be able to be at the facility for more hours. The Director acknowledged this requirement and stated that the documents will be gathered and sent to the Department as soon as possible. LPA emphasized that Staff #1 and Staff #2 cannot be on the premises as employees working more than then 15 hours per week until they are eligible and show as cleared in the Guardian system. Director understood. LPA Perry provided the Director with a copy of LIC 9182 Criminal Background Clearance Transfer Request form. Director filled it out and provided documents to LPA Perry during visit for S#1. Per facility director she will submit forms for S#2 and submit to department. An exit interview was conducted with facility director. A Notice of Site Visit (LIC 9213) was issued and posted by the Director. A copy of this report and the Notice of Site Visit were provided to the Director with appeal rights.
