What the inspector wrote

Licensing Program Analyst (LPA) Elka Chavez conducted a case management inspection to the above named facility. LPA met with Assistant Director, Denisse Cardenas who assisted LPA with touring the facility. At 12:00 PM LPA toured the facility. LPA did not observe Covid-19 posters throughout the facility. LPA observed 5 of the 7 staff present were not wearing facial coverings inside the facility as recommended by the Los Angeles County Health Department. LPA observed children age 2 and older were not wearing facial coverings inside of the classrooms. At 3:30 PM LPA Chavez provided Covid-19 Technical Assistant to Assistant Director, Denisse Cardenas. Exit interview was conducted with Denise Cardenas. A notice of site visit was given and must remain posted for 30 days.

LPA observed the cabinets in the restroom used to store cleaning supplies in the restroom to be unlocked. Staff #3 stated that the church uses the rooms and they may have removed them. This poses an immediate health and safety risk to children in care.
