Based on record review, the licensee did not comply with the section cited above in 4 out of 5 staff did not have the required Mandated Reporter training on file. Certificates in files were expired, which poses/posed a potential health, safety or personal rights risk to persons in care.

RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

101229 Responsibility for Providing Care and Supervision ... (a) The licensee shall provide care and supervision as necessary to meet the children's needs.(1) No child(ren) shall be left without the supervision of a teacher at any time.This requirement was not met as evidenced by: Based on interviews which were conducted 5 out of 7 staff interviewed validated the allegation. During this investigation 3 out of 7 staff interviewed disclosed child #1 was left unattended for approximately one to two minutes

CHILD CARE CENTER DIRECTOR QUALIFICATIONS AND DUTIES

Based on records and interviews it was determined that Christy Leonard is currently the Assistant Principal for Calvary Chapel Elementary School and is still in the process of becoming the Preschool director.This poses a potential health and safety risk to children in care.potential

CRIMINAL RECORD CLEARANCE

101170(e) (1): Criminal Record Clearance--All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility: Obtain a California clearance or a criminal record etc... This requirement was not met as evidenced by... Based on interviews and review of Guardian Background Check System, Christy Leonard does not have finger print clearance. This poses an immediate health and safety risk to the children in care. $500 Civil Penalty was accessed.

101238 Buildings and Grounds (g) Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children shall be stored where inaccessible to children.The requirement is not met as evidenced by: LPA observed clorox wipes in room 112,113,114,115,116,117 in the classroom sinks used by the preschool children. LPA observed the cabinets in the restroom used to store cleaning supplies in the restroom to be unlocked. Staff #3 stated that the church uses the rooms and they may have removed them. This poses an immediate health and safety risk to children in care.

BUILDINGS AND GROUNDS

101238(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors.The requirement is not met as evidenced by: LPA observed electrical outlets without covers in room 112,113,114,115,116,117 and in the restroom used by the preschool children.
