What the inspector wrote

On 12/23/2025 at 9:35AM Licensing Program Analyst (LPA) Mariah Aguirre conducted an Unannounced Case Management Incident inspection to follow up on an Unusual Incident Report submitted to the department via phone call on 12/15/2025. Upon arrival, LPA met with Director Harvinderpal Vaghela. LPA was led on a tour of the facility. The purpose of the visit was to obtain additional information regarding an incident that occurred on 12/15/2025. Census was taken. LPA observed 74 children with 14 staff. On 12/15/2025 the department received an unusual incident report that was submitted to the department via phone call from the facility pertaining to a child and parent's behavior that led to police being called by the facility. The incident was reported by the Director, who witnessed the incident and reported it to the department within the required 24 hour time frame. Written UIR was received via email on 12/22/2025. Written report was received in a timely manner. During the inspection, LPA conducted interviews with staff, and facility director regarding the incident that took place. LPA MAguirre also obtained a copy of a current children's roster, parent handbook, admissions agreement, and other supporting documents. Based on information that was obtained at the time of inspection and the information obtained via the submitted incident report, a follow up visit will be conducted at a later date as further information is needed. No deficiencies are being cited during today's inspection. An exit interview was conducted with Director, Harvinderpal Vaghela and a copy of this report along with appeal rights were provided. A Notice of Site Visit was provided to Director Harvin Vaghela and must remain posted for 30 days. Failure to adhear to posting requirements will result in an immediate civil penalty of $100.00.
