What the inspector wrote

Licensing Program Analysts (LPAs) Jeanette Estrada and Tyler Reyes conducted an unannounced inspection at the facility. The purpose of today's visit is to conduct an annual inspection and to follow up on an incident that occurred at the facility on 2/26/25. LPAs met with Site Supervisor Angeles Orozco who guided analysts on a tour of the facility. This is a preschool program which consists of two classrooms. The program operates from Monday through Friday 7:00 AM-5:30 PM. The program offers care to children 2 to 5 years old. At this time only Classroom 1 is open. Classroom 2 is closed due to low enrollment. LPAs observed a total of 12 children supervised by 4 staff in Classroom 1. LPAs reviewed criminal record clearances for staff present. Staff 2 has obtained a clearance but is not associated to the facility. A type A citation will be issued today for regulation Criminal Record Clearance 101170(e) Per Site Supervisor, Staff 2 is a substitute from Child Care Careers and has been working at the facility on and off for 2 months. All areas identified on the facility sketch were inspected. Each classroom is separate from the other and has it's own entrance. Furniture and equipment are age appropriate and in good condition in both classrooms. Telephone service, heating, lighting and ventilation were evaluated and are operable. The facility has central air and heating. Lighting was observed to be in operable condition. Children have their own cubby in each classroom to store their belongings. Children are provided with a cot for nap time. Children bring their own blankets and sheets and they are sent home weekly or as needed to be laundered. Per Site Supervisor the isolation area is located in the office. Each classroom has 1 restroom with 2 toilets and 1 sink. There is also a diaper changing table in each restroom. Per Site Supervisor, the program assists children with potty training and parents provide the diapering supplies. Age appropriate sinks and toilets were inspected for availability and good repair in both restrooms. General sanitation was observed. Restrooms are cleaned and disinfected if needed during isolation. There is one additional hand washing sink in each classroom available for children. Availability of indoor drinking water was observed in classrooms via a jug and disposable cups. Per Site Supervisor, the water jug is filled from a filter attached to the hand washing sink. Page 2 Disinfectants and cleaning solutions are stored above the staff hand washing sink inaccessible to children in care. Per Site Supervisor, the staff prepare the cleaning solution and dispose of it daily.Per Site Supervisor, poisons are stored in locked cabinets in the staff restroom and medications are stored in an inaccessible area in the office. Per Site Supervisor, only prescription medication is administered at this facility. Children's roster was reviewed and is current. Parents sign in/out in the classroom. LPAs reviewed the disaster drill log. The last emergency drill was conducted on 11/7/24. The program provides breakfast, lunch and snack to the children. Menus are posted at the beginning of the month on the parent board in the classrooms. LPA inspected the food preparation area (kitchen). The kitchen is inaccessible to children in care. The kitchen area was observed to be clean and free of litter. Food supplies were inspected and did not contain any expired items. Per Site Supervisor, children with allergies are provided alternatives and allergy lists are posted in the classrooms and kitchen. LPAs observed an operable carbon monoxide detector in each classroom. The facility is equipped with a centralized fire alarm system throughout each classroom and the kitchen area and with at least one serviced (1/2025) fire extinguisher. Per Site Supervisor there are no firearms stored on the premises. There are no pools or bodies of water at the facility. Per Site Supervisor, plastic or inflatable wading pools are not used for water play. The outdoor playground area was evaluated. The outdoor area is fully enclosed. Both classrooms have direct access to the outdoor playground. There is a large play/climbing apparatus in the middle of the playground. Outdoor equipment is in a safe condition, free of sharp, loose or pointed parts. All areas around or under high climbing equipment and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Availability of outdoor drinking water is provided via a water jug and disposable cups. Children\342\200\231s Records were reviewed for completeness. Children's records contained all required documents and information. Staff records were reviewed via email. Per Site Supervisor personnel files are kept at the headquarters office. Files reviewed contained the required documents. At least one person has a current pediatric first aid/ cpr certification (Exp. 5/25/26). Site Supervisor was reminded Mandated Reporter training and pediatric first aid/cpr shall be completed every two years. Page 3 During this visit LPAs also followed up with an unusual incident report (UIR) that was submitted to the Department. It was reported that on 2/26/25 Child 1 fell in between a step and a platform on the climbing structure in the outdoor playground. Per interview with Site Supervisor conducted during today's visit, Child 1 was up on the platform and was looking up when they attempted to place their foot on the step in front of them. Child 1 missed the step and fell through the open space. Child fell to the ground and the left side of their head hit the cushioned floor. Per interviews, Staff 1 witnessed Child 1 on the play apparatus when the incident occurred. Staff 1 offered Child 1 an ice pack but child refused. Per interview with Site Supervisor, the incident occurred at approximately 11:25 AM and Staff 1 notified Site Supervisor at 12:50 PM. Site Supervisor advised Staff 1 to call Child 1's authorized representative to notify them of the incident. Staff 1 called parent at approximately 1:50 PM and child was picked up shortly after. Per parent, Child 1 was taken to the doctor and no issues were reported. Per Site Supervisor, no doctor's note was provided by parent. Per Site Supervisor, Staff 1 stated they did not notify her and call parent immediately when the incident occurred because child was not allowing them to check for injuries. Per Site Supervisor the facility policy is to call parents upon the occurrence of an injury above the shoulders. Site Supervisor stated an informal meeting was held with all teaching staff on incident reporting requirements regarding serious injuries. A type B citation is being issued today for regulation Health Related Services 101226(a) due to the parent not being notified immediately when the incident occurred. Facility Representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP . Page 4 This facility provides Incidental Medical Services \342\200\223 IMS. A copy of the report shall be posted. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. A notice of site visit was given to Facility Representative and must remain posted for 30 days. Exit interview conducted. Report was reviewed with Site Supervisor Angeles Orozco. A copy of the report and the Appeal Rights were provided.
