What the inspector wrote

Licensing Program Analysts (LPA) Susann Sanchez conducted a case management - other to amended the 9099 dated on 10/27/25. Report had a name of the agency used during a complaint investigation. Name of the agency was removed during todays visit. The following was also discussed during today's visit: - follow up questions from statement that were collected from staff - Plan of Correction (POC) from 10/27/25. POC was submitted to LPA Estrada on 11/07/25 and an additional copy was given to LPA Sanchez on 1/5/26 via email. LPA Sanchez cleared the POC during inspection. A notice of site visit was issued. Exit interview conducted with Teacher Karen Ramirez. A copy of this report was provided.

    (a) The licensee shall ensure that each child is accorded the following personal rights: (3) To be free from...infliction of pain...other actions of a punitive nature including but not limited to: interference with functions of daily living including...sleeping. This requirement was not met as evidenced by Based on interviews S1 &S2 witnessed a sub staff put their leg on a child during nap time while the child stated they couldn't breathe. Sub Staff was immediately told to stop. This poses an potential risk to the safety and personal rights of children in care.
