What the inspector wrote

On 5/28/2025, at 08:30 am , Licensing Program Analyst(LPA) Shushanik Safaryan conducted an unannounced POC (Plan of Correction) inspection to the above facility to ensure Type A deficiency cited on 05/12/2025 has been cleared. Upon arrival LPA met with Haykanush Baroyan , licensee who guided analyst on a tour of the facility. During this inspection 8 children were present in the home. Licensee’s mother was present too. During the visit LPA toured outdoor area where in-ground pool is located. LPA observed gate was not obscuring pool from view. LPA observed Plexiglas on the gate. Gate was swinging away from the pool and was self-close with self-latching device. Device was located no more than six inches from the top of the gate . LPA observed lock on the gate that was locked with the key. Next LPA observed door alarm on the gate , which sounded operable when licensee opened the door. LPA measured side openings which measured less than 3 inches from both sides. Next LPA measured opening from the bottom to the gate which was less than 2 inches. LPA observed pool alarm in the pool. Licensee tested pool alarm which sounded operable. During the visit, LPA cleared deficiency cited on 05/12/2025 and provided a copy of the Licensing Report to Licensee and issued POC clearance letter. The Notice of Site Visit (LIC 9213) was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. End of the report

LPA reviewed pictures of the gate on the file which is different from the existing gate .LPA observed different gate which did not match pictures on the file . Licensee stated she made a change in 2024 and gate was altered.
