What the inspector wrote

On 05/28/2025, at 9:30 am Licensing Program Analyst (LPA) Shushanik Safaryan conducted an unannounced case management inspection to the above facility . Upon arrival , LPA met with the Haykanoush Baroyan , Licensee who guided LPA on the tour of the home. During the visit, LPA observed gate to the in-ground pool was different from the one on the record that was approved by the Department . LPA interviewed licensee regarding the change. Licensee stated she did not know that it needs to be reported . LPA provided licensee with the regulations and obtained declaration from licensee stated she understands that all the changes need to be reported to the Department. Based on the LPA’s observations and records review, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety. LPA discussed all the deficiencies cited during today’s inspection with the licensee. Per licensee, she understands all the deficiencies and plan of correction . A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report , appeal rights was reviewed and provided to the licensee Haykanoush Baroyan on 05/28/2025.

LPA reviewed pictures of the gate on the file which is different from the existing gate .LPA observed different gate which did not match pictures on the file . Licensee stated she made a change in 2024 and gate was altered.
