What Happened

Complaint investigation substantiated. Facility failed to prevent spread of illness during diarrhea outbreak. Multiple children with diarrhea were not fully isolated, and one child was readmitted the day after being sent home ill, violating the facility's own 24-hour symptom-free return policy.

Observations

  • Multiple children with diarrhea during facility outbreak separated to different table but not fully isolated from well children
  • Sick room available but not utilized due to inadequate staffing and lack of facilities for diarrhea care
  • Child sent home with diarrhea at 1:00 PM not picked up until 5:00 PM, remaining in classroom with other children
  • Child documented with diarrhea on two consecutive days despite facility policy requiring 24-hour symptom-free period before readmission
  • Staff unable to maintain complete isolation due to staffing constraints and inadequate bathroom access for diarrhea cases
