Case management incident investigation following child's right femur fracture on 10/8/2025 during PM recess. Investigation found facility supervision plan was not followed, yellow slide area was left unsupervised, and staff assigned to area was not positioned to observe slide per guidelines. Type A citation issued indicating immediate risk to health and safety.

Annual inspection of preschool facility identified epi-pen improperly stored without original packaging or identifying information. Lead testing completed with no exceedances but results not posted to department database.

Complaint investigation substantiated. Facility failed to prevent spread of illness during diarrhea outbreak. Multiple children with diarrhea were not fully isolated, and one child was readmitted the day after being sent home ill, violating the facility's own 24-hour symptom-free return policy.

Type A citation issued for inadequate supervision during biting incident. Child #1 bit Child #2 causing bleeding; incident occurred because staff were not watching the children despite prior incidents.

The preschool program was operating in ratio and under visual supervision, but the inspection found cots stacked in a way that created unsafe bedding contact and uncovered food items with no labels or expiration dates. A sign-in/out issue and missing teacher qualification documentation were also noted.

A complaint inspection of the infant and preschool licenses found three preschool staff working who were not associated to the facility roster. The facility was cited, and the report required immediate parent notification and posting of the Type A citation.
