What the inspector wrote

Licensing Program Analysts (LPA)s Stephanie Li and Mariah Aguirre conducted an unannounced Case Management inspection due to an incident that occurred on 9/17/2025 and was reported to the Department. A risk assessment was conducted. LPA met with Director Arpine Patatanyan, to whom the reason for the visit was explained. Director guided LPA on a tour of the facility. There were 47 children present and 9 staff. On 9/17/2025, an unusual incident report was made to the Department regarding an incident where Directors own 1 st grade child went into the play yard and the door closed and was locked from the outside. Child got locked out of building. Bystanders observed child crying from parking lot and ringed the front gate bell. Director opened the gate and was informed about her child. She immediately opened the back door for him and he came back inside the building. Facility reported to the incident and submitted video recordings of incident. During todays visit, LPA interviewed Director regarding the incident, toured the facility, and verified and obtained copies of sign in and out via brightwheel for the day of the incident. During the tour, LPA\342\200\231s observed Directors infant child in off limits kitchen area with extra assistant staff. LPA advised that infant should not be in the off limits kitchen for health and safety best practice and to use the unused room for infant with extra staff during time infant needs to be at the facility. LPA\342\200\231s advised and explained to Director Arpine that her own children that are not of the facility licensed ages cannot be in the same room as the center\342\200\231s enrolled children and must be supervised at all times. Due to the ages of the directors children (1 st grade and infant) and facility children ages, Directors children cannot co-mingle with licensed children. LPA\342\200\231s advised that an unused room may be designated for her own children to be in when she needs to bring them to work. Per director, she only needs to bring her children to the facility every once in a while and they do not stay long. Per Director, her husband picks the kids up from here. Director showed LPA\342\200\231s a small room connected to the lobby that can be designated for her son to use if he has to be at facility. Page 1 of 2 Based on information obtained during this investigation, no follow up is necessary regarding the incident reported. Child is not enrolled at facility so no deficiencies are being cited at this time. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Director Arpine Patatanyan. pg 2 of 2
