Correction check·March 4, 2026

On 3/4/26, at 1:45PM, Licensing Program Analyst (LPA) Joe Katrdzhyan conducted an unannounced Case Management Inspection – Plan of Correction at the above facility. Upon arrival, LPA met with Staff / Liana Hovhannisyan and was joined by the Licensee / Temine Atoian shortly after, who guided LPA on a tour of the facility. The LPA observed 14 children present during today's inspection. Per Licensee, there are currently 33 children enrolled. The purpose of this inspection is to ensure that the facility is in compliance with Title 22 Regulations and the deficiencies cited on 2/11/26 and 2/18/26 were corrected. LPA observed and reviewed the following: LPA received supporting documentation for the plan of correction related to Staff members 1 and 2 missing the TDAP and MMR Vaccines and Volunteer 1 missing the TDAP, MMR and TB Vaccines . The plan of correction (POC) was submitted to Community Care Licensing on 2/23/26. The deficiency has been cleared, and no further action is required. LPA received supporting documentation for the plan of correction related to the Licensee operating a Family Child Care Home beyond the conditions and limitations specified on the license, including the capacity limitations. The plan of correction (POC) was submitted to Community Care Licensing on 2/19/26. The deficiency has been cleared, and no further action is required. LPA received supporting documentation for the plan of correction related to the Licensee operating a Family Child Care Home and providing care for four (4) infants and eleven (11) preschool aged children at one time. The plan of correction (POC) was submitted to Community Care Licensing on 2/19/26. The deficiency has been cleared, and no further action is required. Letter of Deficiency Citations Cleared was provided for the deficiencies corrected. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. An exit interview was conducted, and a copy of this report was provided to Licensee / Temine Atoian.
