Inspector’s narrative

What the inspector wrote

During inspections on 02/07/2024, 02/23/2024, and today's date, 03/07/2024. LPAs observed that there were multiple electrical outlets without covers on them in the classrooms. This makes the outlets accessible to children in care. Pictures were obtained. Director states that they were provided 15 Amp Tamper Resistant AFCI Outlets from their contractors and submitted the receipt to LPA. Outlet on receipt only showed quantity of 2 ordered and did not match any outlets in the classrooms observed. Based on LPAs observations and interviews which were conducted and record review(s), the preponderance of evidence standard has been met, therefore the above allegation(s) is found to be SUBSTANTIATED. California Code of Regulations, (Title 22, Division 12 Chapter 1 101238(b)(1)), are being cited on the attached deficiencies page. The deficiencies listed on the following pages were observed by the LPA and are being cited in accordance with California Code of Regulations Title 22. Please see attached LIC 809D for deficiencies that are being cited and need to be cleared to protect the children’s health & safety. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director Monique Ornelas, at 9:45am. Plan of corrections developed and Copy of Report provided. END OF REPORT PAGE 2 of 2 Interviews with staff indicated that toys and equipment are checked multiple times throughout the week for any broken or hazardous parts. Staff interview states that toys that are broken or not age appropriate have been brought up to administration. Director states that Licensee is attentive in replacing any equipment or toys that are not considered safe. Parent interviews made no disclosures regarding the above allegation. During inspections on 02/07/2024, 02/23/2024, and today's date, 03/07/2024, LPAs made no observations of toys or equipment that were broken, unsafe, or not age appropriate for children in care. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation is unsubstantiated. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director Monique Ornelas, at 9:45am. Copy of Report provided.
