This requirement was not met evidenced by: Multiple incdents were recorded in the area where uneven pavement and sand pit . LPA observed incident reports in the children`s files and verified with staff place of the incident. Per staff uneven pavement is the area where most of the incidents happened. Disclosures were made that it could be at least 1-3 incidents per week .
