What the inspector wrote

Licensing Program Analyst (LPA) Stephanie Li conducted an unannounced Case Management Inspection to the above facility. Upon arrival LPA met with the assistant director, Jackie Hernandez, who guided LPA on a tour of the facility . During the visit LPA observed 36 children with 9 staff members . During the visit, LPA conducted staff file reviews, staff interviews and obtained LIC500 and LIC9040, and staff schedules. Based on the information reviewed, assistant director is missing 3 units of Administration or staff relations course. Per Title 22 Section 101215.1(h)(1)(a) child care center director qualifications, three of the 15 units required in (h) (1) above shall be in administration or staff relations. Based on the information obtained the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00 Exit interview and report reviewed with Assistant director Jackie Hernandez. Appeal rights were provided.

  • RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

    (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation.This requirement was not met evidenced by: Based on interviews and record review, one child was left outside in the play yard alone without adult supervision for 15-20 minutes, which poses an immediate risk to the health and safety of children in care.

    101215.1(h)(1)(a) child care center qualifications, three of the 15 units required in (h) (1) above shall be in administration or staff relations.This requirement was not met evidenced by: Based on interview and record review, assistant director is missing 3 units of administration or staff relations, which poses a potential health and safety risk to children in care.
