What the inspector wrote

On September 22, 2025, Licensing Program Analyst (LPA) Monique Ayala conducted an unannounced Case Management- Incident inspection at the above facility. LPA met with director, Emily Laguna who guided LPA on a tour of the facility. LPA observed 26 children in care with 7 staff members. The purpose of the inspection is to follow up on an incident that occurred on 09/16/2025. During the inspection, LPA interviewed Staff #1 (S1) to Staff #2 (S2), obtained a facility roster, inspected the area where the incident occurred, LPA attempted to interview Child #1 (C1), C1 refused and LPA attempted to interview Parent #1 (P1) but there were no answer. LPA left call back information. Based on interviews, S1 stated they took C1 to the restroom to assist with a change in pants. Per S1, S1 was assisting C1 with putting on their new pants (change of clothing). Per S1, C1 started to lose their balance while placing their foot in the pant leg causing C1 to fall backwards. Per S1, C1 fell backwards causing C1 to hit the back of their head on the floor. Per S1, there were no other children or staff in the restroom at the time of the incident. Per S1, they immediately checked C1's head, comforted C1 and applied an ice pack. Per S1, S2 and assistant director heard C1 cry and immediately arrived to the restroom to offer assistance. Per S1, they called both of C1 parents and sent a message through the Brightwheel App. Per S1, C1 was not taken to seek medical attention and arrived at the facility the following day. At this time the facility is not being cited any deficiencies. The incident appears to be an accident the occurred quickly and was unable to be prevented. LPA informed the director to ensure that the floors in the restroom are not wet before and after usage. An exit interview was conducted and a copy of this report was provided and must be posted for 30 days.
