What the inspector wrote

On October 14, 2025, Licensing Program Analysts (LPAs) Monique Ayala and Mariah Aguirre conducted an unannounced Case Management inspection at the above facility. LPAs met with assistant director, Jackie Hernandez who guided LPAs on a tour of the facility. LPA observed 15 children in care with 3 staff members. The purpose of the inspection is to follow up on an incident that occurred on 10/10/2025 and was reported to the department on 10/10/2025; report was reported timely. During the inspection, LPAs obtained a copy of the facility roster, LPAs interviewed Staff #1 (S1), Staff #2 (S2) was not present at the time of inspection and an interview could not be conducted. LPAs were unable to interview Child #1 (C1) but C1 was not present at the facility. Based on interviews, S1 stated that C1 sat on the far back of the bike, resulting in the bike tipping backwards and C1 falling off the bike. After the incident took place, S1 stated S2 examined C1's head, and scrapes were present due to the fall. S1 stated S2 applied an icepack to C1's head and an incident report was written and given to C1's parents. Per S1, they were in ratio at the time the incident occurred. At this time, no deficiencies are being cited. An exit interview with Assistant Director Jackie Hernandez was conducted and a copy of this report was provided. A notice of site visit was provided and must remain posted for 30 days.
