On October 14, 2025, Licensing Program Analysts (LPAs) Monique Ayala and Mariah Aguirre conducted an unannounced Case Management inspection at the above facility. LPAs met with assistant director, Jackie Hernandez who guided LPAs on a tour of the facility. LPA observed 15 children in care with 3 staff members. The purpose of the inspection is to follow up on an incident that occurred on 10/10/2025 and was reported to the department on 10/10/2025; report was reported timely. During the inspection, LPAs obtained a copy of the facility roster, LPAs interviewed Staff #1 (S1). LPAs were unable to interview Child #1 (C1) as C1 did not want to be interviewed. Based on interviews, S1 stated C1 was lined up to wash hands, and walked out of the line to run around with another child (C2). When getting out of line to run, C1 bumped into C2 and fell face first onto the ground. When S1 helped C1 up, a bump was present on C1's forehead and blood was present on the nose and mouth. Per S1, they took C1 to the first aid kit and began administering first aid, providing C1 with a gauze to stop the bleeding present on the inside of C1's mouth. S1 brought C1 inside and parents were contacted. Per S1 an incident report was written and provided to C1's parents. Per S1, they were in ratio at the time the incident occurred. At this time, no deficiencies are being cited. An exit interview was conducted with Assistant Director Jackie Hernandez, and a copy of this report was provided, a Notice of Site Visit was provided and must remain posted for 30 days.
