Incident investigation·November 3, 2025

On November 3, 2025, Licensing Program Analyst (LPA) Monique Ayala conducted an unannounced Case Management- Incident inspection at the above facility. LPA met with director, Emily Laguna who guided LPA on a tour of the facility. LPA observed 32 children in care with 9 staff members. The purpose of the inspection is to follow up on an incident that occurred on 10/21/2025 and was reported to the department on 10/21/2025. The incident was reported timely. During the inspection, LPA interviewed Staff #1 (S1) and Child #1 (C1). LPA obtained relevant documentation. At this time the above incident requires further investigation as LPA was unable to interview Staff #2 (S2) and Child #2 (C2) as they were not present at the facility. There are no deficiencies being cited at this time. An exit interview was conducted and a copy of this report was provided to director. A Notice of Site Visit was provided and must be posted for 30 days.
