What the inspector wrote

On April 28, 2026, Licensing Program Analyst (LPA) Monique Ayala conducted an unannounced case management inspection at the above facility. LPA met with director, Emily Laguna who guided LPA on a tour of the facility. LPA observed 29 children in care with 6 staff members. The purpose of the inspection is to follow up on three incident reports the facility reported to the department. Incident report 1: occurred on 02/11/2026 to 02/23/2026 where four children were diagnosed with an upper respiratory infection. Per director, the 4 children have returned to the facility and there were no additional cases. Incident report 2: occurred on 02/27/2026, where a child was sliding down the slide face forward causing the child to hit their forehead on the pavement. LPA observed the slide to be age appropriate and informed the director that children should be redirected to slide down the slides correctly. Per director, there were no injuries (cuts), an ice pack was provided and the child was observed to ensure there were no signs of concussion. Director stated child's parent was called and informed of the incident. Incident report 3: occurred on 04/20/2026, where a child was walking and when the child turned their head, hit the edge of the wall. The child sustained a small cut with bruising on their forehead. Per staff #1, they observed the incident, cleaned the area and applied ice. Per S1, parents were called and informed of the incident. Per S1, child was not taken to seek medical attention and returned to the facility the following day. LPA observed the wall where the incident occurred, LPA did not observed sharp edges. There are no deficiencies being cited at this time. An exit interview was conducted and a copy of this report was provided to the director, Emily Laguna. A Notice of Site Visit was provided and must be posted for 30 days.
