ANNUAL/RANDOM INSPECTION CONDUCTED IN ARMENIAN
Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced Annual/ Random inspection to the above facility on 03/06/26. LPA arrived at the facility at 8:40 AM and met with licensee, Anna Vardanyan who guided analyst on a tour of the facility. During this inspection licensee's assistant, Azatuhi Narinyants and licensee's daughter/assistant, Eliza Vardanyan were present in the home. LPA provided a copy of Entrance Checklist\342\200\224Family Child Care Home (LIC126) to licensee. Per licensee, facility operation hours are Monday to Friday, 7:00 a.m. to 10:00 p.m.., Licensee states she cares for children 0-14 years old.
This is a one story home located on the first level. The home consists of 3 bedrooms, 2 & 1/2 restrooms, living room, dining room (main daycare area), kitchen, attached garage, front yard, side yard and back yard (fenced). Per licensee, parents enter the home through the side gate which is located on N. Maryland Ave., and leads to the side yard and dining room (daycare area).
Per licensee the children use the half bathroom in the dining room, dining room (main daycare area), side yard, some part of back yard (fenced). Per licensee areas off limits to children and parents include: Living room, three bedrooms, two bathrooms, kitchen, garage, front yard and back of backyard. LPA did not observe any stairs in the home during this inspection. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report.
All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. At 8:45 a.m., Licensee took this LPA on a tour of the home. LPA was toured through the interior starting with the dining room (main daycare classroom), LPA observed children tables,

chairs, toys, napping cots and a TV in the dining room. During this inspection LPA observed 12 children were playing in the dining room with licensee and her assistants. LPA observed all electrical outlets were covered in dining room (main daycare area). Next LPA toured the half bathroom in the dining room and did not observe any hazards. There is a door in the dining room (daycare room) which leads to the off-limit living room and rest of the off-limit areas in the home. Per licensee, she keeps the daycare door closed all the time to keep the off-limit areas inaccessible to the children. There are two doors in the dining room which leads to the backyard (fenced) daycare area and side yard (fenced). Per licensee, she keeps the required 2A 10BC fire extinguisher on the floor in the kitchen. Licensee brought the fire extinguisher to the daycare area. It was purchased on 02/05/26 as read by LPA from the receipt. Licensee tested the carbon monoxide and smoke detectors in the dining room (daycare area). It sounded off the alarms and heard by LPA to be functional. Detergents, cleaning compounds, medications, and other items which could pose a danger to children were observed to be inaccessible to children. The licensee states that there are no poisons in the home and understands that storage areas for poisons must be locked with a key or combination lock.
Next LPA toured the backyard, and side yard fenced. Per licensee, there are three outdoor playareas in the home for children. Per licensee, children use the back yard area adjacent to the kitchen and dining room, side yard (fenced) for outdoor play time. Per licensee, children enter the backyard daycare area through the door from the dining room. Per licensee, children enter the side yard through the dining room. Per licensee, children line up and under supervision walk out through the side yard adjacent to the dining room to the drive way to enter the side yard (fenced) which is located toward the Stocker street and Maryland Avenue. LPA observed three outdoor play areas were fenced. Next LPA toured the back of backyard (off-limit area) and did not observe any hazards. LPA observed off-limit backyard area is separated by six feet high gates. LPA observed that the outdoor yard has toys, swings and other materials for children to play with. LPA did not observe any objects that can pose a danger to children on the outdoor yard. The licensee states that supervision is always provided.
Licensee OWNS THE HOME: The licensee provided proof of control of property.
Per licensee, she provides food for children in care. Licensee was advised that if food is brought from the children\342\200\231s homes, all containers must be labeled with child\342\200\231s name and properly stored or refrigerated. Per Licensee, Anna Vardanyan at 9:30 a.m there are no pets, weapons, firearms, pools, spas, hot tubs, fish ponds, or similar bodies of water on the premises.

The licensee was observed to be operating within the licensed capacity and is not exceeding the required limitations during this inspection. During this inspection 12 children were present in the home with licensee and her assistant. All adults present have obtained a criminal record clearance on Licensing Information System (LIS). Per licensee, Anna Vardanyan at 10:00 a.m. currently she has 2 assistants. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
The licensee and other personnel have completed training on preventive health practices including Pediatric First Aid and CPR. The licensee's Pediatric First Aid and CPR expires on 02/2027.
Licensee, Anna Vardanyan completed required mandated reporter training on 02/12/25. There are first aid supplies available on the premises in the dining room (main daycare area). LPA advised licensee that if a child shows signs of illness he/she/they shall be separated from other children.
At 10:15 a.m., facility roster was reviewed and completed. Per licensee currently there are 14 children enrolled. LPA reviewed 6 children's records, including emergency information from 14 enrolled children, and all 6 reviewed files were completed.
The licensee and her assistants have proof of immunization against influenza, pertussis, and measles. All homes shall conduct fire and disaster drills at least once every six months, and document the date and time of each drill. Last drill documented was conducted on 10/20/25. Emergency Disaster Plan, Parent\342\200\231s Rights Poster and the Facility License were observed to be posted at the wall by the entrance in the dining room (main daycare area).
Smoking is prohibited in a licensed Family Child Care Home. Per Licensee, no one smokes in the home. There is telephone service via a cellphone that is used and the cellphone stays at the facility during operation hours.

Per licensee she does provide carries liability insurance in accordance with standard established by Family Child Care statue.
Per licensee she works with food program. Per licensee, she does not provide transportation.
INFANT CARE: Licensee states that she provides care to infants, but currently she does not have an infant enrolled in her daycare. Per licensee, if an infant is enrolled, the infant will sleep in the dining room (main dayare area) where they are constantly supervised. LPA advised the Licensee that infants shall be placed on their backs for sleeping and shall be supervised. Infants shall be checked on every 15 minutes and the time of each 15-minute check shall be documented with child\342\200\231s name and date. The LIC 9227 Individual Infant Sleeping Plan shall be completed for each infant up to 12 months of age. A copy of the LIC 9227 was provided to Licensee.

as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at

and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
Medication:

LPA advised the licensee to access forms, regulations and quarterly updates online at:

At this time, the licensee is in compliance with California Title 22 Regulations. Therefore, there are no citations being issued today from the areas that were inspected today.

Licensee, Anna Vardanyan was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
During the exit interview, the Licensee, Anna Vardanyan, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
