On September 30, 2025, at 1:35pm, Licensing Program Analyst (LPA) LPA Keneisha Dunlap arrived at the above facility for an unannounced case management visit for POC. LPA Dunlap announced the purpose of the visit and was granted entry into the facility by Licensee - Sonia Silveyra. There are 4 children enrolled, 3 children present, and 1 staff present at the time of inspection. This is a preschool program, and the center hours of operation are Monday- Friday 6:00am-6:00pm. On September 19, 2025, the facility was cited for the following deficiencies: Staff Records - Type B: 1596.8662(b)(1) - 1 out of 1 staff member did not have Mandated Reporter Training certificate. This deficiency has been cleared as off September 30, 2025. LPA Dunlap reviewed staff records with a current Mandated Reporter Training certificate with an expiration date of 1/23/26. Staff Records - Type B: 101216(f) - 1 out of 1 staff member did not have a CPR and First Aid certificate. This deficiency has been cleared as off September 30, 2025. LPA Dunlap reviewed staff records with a current CPR & First Aid certificate with an expiration date of 2/8/27. Children Records - Type B: 101221(b)(6) - 1 out of 1 children file did not have an admissions agreement. This deficiency has been cleared as off September 30, 2025. LPA Dunlap reviewed the admissions agreement. All parents have signed documents, and the admission agreement has been placed in the children\342\200\231s files.
