Case Management visit to deliver deficiency report. Infants observed sleeping with blankets in play yards, violating safe sleep regulations. Deficiency cited. Licensee interested in Technical Support Program (TSP).

Complaint investigation substantiated allegation that licensee hit children, violating personal rights. Type A deficiency issued.

Case management visit for amended report signature. Facility found out of ratio with 8 children (5 infants under 2, 3 children age 2) and only 1 staff member present. Two overnight infants picked up late by authorized representatives. Type A deficiency cited for ratio violation as repeat violation with $250 civil penalty assessed.

Two overnight children were picked up late (after 9am when required pickup time was 9am), resulting in a Type B citation for supervision violation.

Case Management visit found critical violations: facility operating over licensed capacity (17 children vs 14 licensed), staff without fingerprint clearance left alone with 12 children at park, and licensee failed to verify staff clearance before employment. Three Type A deficiencies and one civil penalty (LIC421IM) issued.

Complaint inspection revealed uncleared staff member working at facility, attached pacifiers on three infants, operation over capacity with uncleared assistant present, and overdue licensing fees. Multiple Type A and Type B deficiencies with civil penalty assessed.

The annual large family child care inspection found the home operating with multiple infants and older children, but staff immunization records were incomplete. Safety systems were in place, and the home maintained safe sleep equipment, outdoor play space, and current record forms for the children.
