What the inspector wrote

On 10/09/2024 at 10:30am, Licensing Program Analysts (LPAs), Elka Chavez and Crystal Green conducted an unannounced Case Management inspection for an increase in capacity. Upon arrival, LPA met with Facility Representative Veronica Tapia. This facility is currently licensed as a preschool program with a capacity of 61 children serving ages 2 to 6 years old in (3) classrooms: Binomial Cube, Bead Stairs, Knobbed Cylinders, 24 infants ages 12 to 24 months in (1) classroom: pink tower and 24 toddlers ages 18 to 36 months in (1) classroom: color tablets. The Licensee, M.A.O.D, Inc., has requested to increase the preschool capacity by closing the existing toddler component with a capacity of 24 toddler in (1) classroom: Color Tablets and adding it to the existing preschool licensed areas for a total new capacity of 145 preschool children that will be operating out of (4) classrooms. Hours of operation will be Monday through Friday from 7:00am to 6:00pm. Present during today\342\200\231s inspection were 79 children with 14 staff. Appropriate ratios and supervision were observed. All indoor and outdoor activity space utilized for the children was inspected today. LPAs informed Veronica Tapia that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. LPAs continued to tour the facility and measured new additional indoor and outdoor activity space. Total preschool indoor activity space measured 3930.41 square feet, which is insufficient for the requested capacity for preschool children. Total infant indoor activity space measured 590.75 square feet, which is sufficient to accommodate the requested capacity of 16 infant children. LPAs observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots/mats, bookshelves, and other activity supplies for the children. Drinking water is available in the classrooms via Brita water pitcher. LPAs observed one crib is kept on premises in case an infant needs it. LPAs observed a changing table within arm\342\200\231s reach of a sink in the pink tower (infant) classroom. LPAs were informed that meal preparation is conducted in the kitchen. Applicant representative stated that they are in the process of purchasing a portable sink, shelf and cabinets to be able to prepare meals inside the pink tower classroom. LPAs observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted on 09/24/2024. LPAs observed a total of 14 sinks and 12 toilets available for children\342\200\231s use. These are sufficient to accommodate the requested capacity of 112 children. LPAs did not observe any potty chairs in the pink tower classroom. The facility currently has a fully fenced playground area. There are two designated program outdoor areas: Infant and Preschool Program. The facility would like the preschool children ages 2 to 3 to use the infant playground. A waiver to utilize the designated infant yard using a staggered play schedule was collected during the inspection. Fencing was observed to be white concrete wall and wrought iron and is at least four feet high. The total square footage for the designated infant outdoor activity space is 1,770.66 square feet, for only 23 children at one time which is sufficient for the capacity requested. LPAs observed adequate shade in the infant playground. The total square footage for the designated preschool outdoor activity space is 5643.64 square feet, for only 75 children at one time, insufficient for the number of children being requested. A playground waiver to share the playground at different times in the AM and PM due to limited square footage shall be submitted and approved prior to licensure. Shade is provided via patio cover and tress. There are sufficient outdoor age-appropriate toys and play equipment available on the playground. There is a climbing structure located on the designated preschool playground for children ages 2 to 12 years of age which is properly anchored. LPAs observed parts of the structure were not appropriate for younger children. LPAs observed the structure has a step pole with approximately 18 inches away from the platform to the ground, spiral ladder with approximately 20 inches away from the platform to the ground and a spiral pole with approximately 16 inches away from the ground to the platform. LPAs observed the area inaccessible to children in care via clear plastic tarp barriers. There is adequate cushioning in fall zones of climber provided by rubber matting. Drinking water is available via Brita water and disposable cups. LPAs observed all hazardous items on the playground to be inaccessible to children. Applicant representative Veronica Tapia was reminded that any changes to the facility must be reported to and approved by Community Care Licensing. The following corrections are needed prior to the issuance of the license: \342\200\242 Applicant needs 3 potty chairs for the infant classroom. Licensee Representative, Veronica Tapia understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied.
