What the inspector wrote

Licensing Program Analysts (LPAs) Brannon and Zamudio met with site supervisor, Maria Ramirez. During today's inspection, LPAs took a census, interviewed staff, reviewed children's and staff files. LPAs observed 12 preschool children with three staff. LPA received an Unusual Incident Report. The incident occurred on 2/8/22. Staff observed child #1 with bruising. Child #1 and Child #2 were admitted into the program as of 11/15/21. On 2/8/22, staff observed that child #1 had a bruise. Staff notified Child Welfare Services were contacted. Staff completed the Suspected Child Abuse Report (SCAR). Madera PD, Social Worker, Public Health Nurse and CAP of Madera Co Health Specialist arrived at center on 2/9/22. At this time, LPA Brannon will review documentation with North Fresno Regional office. A return inspection will be conducted at a later time. Exit interview conducted and report was reviewed with Health Specialist, Patricia Almanza. This report shall be made available to the public upon request. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
