What the inspector wrote

On December 12 2025 Licensing Program Analyst (LPA) Naves conducted an unannounced Case Management visit to the facility mentioned above. LPA met with the licensee Beatriz Chan Rosado. The purpose of the visit was explained and was granted entry into the home. Present during the visit was licensee and 3 helpers supervising 7 children (2 infants and 5 preschool age) children. The facility self reported an Unusual Incident Report (UIR) that occurred on 11/25/2025. A child wandered away from the group while out for a walk and was later found to have been in an apartment across the street. According to licensee Police were called and when Police arrived they were pointed to a building where child was located and found to be safe with his father. LPA Jaclyn Naves informed licensee Beatrice Chan Rosado that this report dated December 12, 2025, document(s) 1 Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Jaclyn Naves informed the licensee Beatrice Chan Rosado to provide a copy of this licensing report dated December 12, 2025 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224 ), or other written statement, must be placed in the child's file for verification. Appeal rights given. Exit interview conducted and report was reviewed with the licensee Beatrice Chan Rosado.
