What the inspector wrote

Licensing Program Analyst (LPA) Robert Maciel made a case management visit to the facility for the purpose of following up on an Unusual Incident Report that was self-reported to the department on 4/8/25 and met with Facility Representative Rachel Scoggins (S1). As reported in the Unusual Incident Report, on 4/8/25 at around 10:30 AM while children were playing outside in the playground, a child (C1) opened a gate leading to the parking lot and ran out before staff could catch him. During today's inspection, the facility was toured and LPA spoke with staff. LPA observed that the gate that C1 had run out of was locked with a padlock. S1 stated that on 4/8/25 a child (C1) attempted to run to the gate leading to the parking lot to leave. S1 observed C1 running and attempted to catch him before he made it to the gate. C1 ran to the gate, opened the latch, and ran out while S1 was chasing him. C1 ran into the parking lot and entered his mother's car before S1 could reach him. At that time there were 2 teachers present supervising 15 children in the playground. The following deficiencies were observed during today's visit (see LIC809D). Exit interview given and report was read with Facility Representative Rachel Scoggins. A notice of site visit was given and must remain posted for 30 days. Failure to do so shall result in an immediate civil penalty of $100.
