Based on an unusual incident report and director statement, on 4/17/26, a child left the facility playground without staff being aware at 10:30 a.m. and was found by a teacher from the elementary school that the facility is located at, at 10:34 a.m. which poses a potential health, safety or personal rights risk to persons in care.
