What the inspector wrote

Licensing Program Analyst (LPA) Robert Maciel made a case management visit to the facility for the purpose of following up on two Unusual Incident Reports that were self-reported to the department on 4/24/25 and 5/1/25 and met with Facility Representative Rachel Scoggins (S1). As reported in the first Unusual Incident Report, on 4/24/25 at around 10:05 AM while children were playing outside in the playground, a child (C1) wandered onto the playground on the Calpella Elementary School playground, adjacent to the facility through the gate behind the transitional kindergarten building, which is not directly observable from the playground. Staff noticed C1 at 10:10 AM who was brought back in by staff. As reported in the second Unusual Incident Report, on 5/1/25 at around 10:40 AM while children were playing outside in the playground, two children, child 1 (C1) and child 2 (C2) wandered onto the playground on the Calpella Elementary School playground, adjacent to the facility through the gate behind the transitional kindergarten building, which is not directly observable from the playground. Staff noticed both children at 10:45 AM who were brought back in by staff. During today's inspection, the facility was toured and LPA spoke with staff. LPA observed that there was a mesh retractable gate installed to prevent access to the area behind the transitional kindergarten building. S1 stated that on 4/24/25 at 10:10 AM, C1 was spotted on the elementary school playground and brought in by staff. S1 was not outside when the incident had occurred but was told by staff who were present that they believed that C1 had eloped through the gate behind the transitional kindergarten building while unobserved. At that time there were 2 teachers present supervising 11 children in the playground. Continued on LIC809-C S1 stated that on 5/1/25 at 10:45 AM, C1 and C2 were spotted on the elementary school playground and brought in by staff. Staff told S1 that during staff transition, both children had eloped through the gate behind the transitional kindergarten building. At that time there were 2 teachers present supervising 11 children in the playground. On Friday, 5/2/25, maintenance staff installed a retractable mesh gate so that children could not access the area behind the transitional kindergarten building. The following deficiencies were observed during today's visit (see LIC809D). Exit interview given and report was read with Facility Representative Rachel Scoggins. A notice of site visit was given and must remain posted for 30 days. Failure to do so shall result in an immediate civil penalty of $100.

Supervision shall include visual observation.This requirement is not met as evidenced by: Based on two Unusual Incident Reports submitted by the facility and interviews from staff, two children (C1, C2), eloped from the playground into the Calpella Elementary School playground without being observed which poses a potential health, safety or personal rights risk to persons in care.
