What the inspector wrote

Licensing Program Analyst (LPA) Robert Maciel made a case management visit to the facility for the purpose of following up on an unusual incident notification that was self-reported to the department on 4/21/25 and met with Facility Representative Rachel Scoggins (S1). As reported in the unusual incident notification, on 4/20/25 at around 12:30 AM during circle time, an adult (S2) under the supervision of facility staff had stepped on a child's (C1) foot in retaliation for the child stepping on hers. During today's inspection, the facility was toured, LPA spoke with staff, and LPA reviewed the written unusual incident report with S1. S1 stated that on 5/20/25 at around 12:30 PM, a child (C1) was not wanting to join circle time and went away to play with blocks. C1 became upset when a staff (S2), asked him to rejoin circle time with the rest of the children. C1 stomped on S2's foot and in response, S2 stepped on his foot, saying "See, ouch that hurts." C1 laughed in response. S2 was immediately switched with another staff to help with C1. The following violation of the California Code of Regulations, Title 22, were cited during today's inspection, (see LIC809-D). Appeal rights were provided. Exit interview given and report was read with Facility Representative Rachel Scoggins. A notice of site visit was given and must remain posted for 30 days. Failure to do so shall result in an immediate civil penalty of $100.
