BUILDINGS AND GROUNDS
Buildings and Grounds. The child care center shall be clean, safe, sanitary and in good repair at all times.This requirement was not met as evidenced by:LPA observed a bookshelf in the school age classroom that was wobbly. Additionally, staff admitted that the bookshelf had previously fell on top of a child. According to staff, child was not seriously injured, an incident report was completed, and the child's parent was notified.

Reporting Requirements: Upon the occurrence, during the operation of the child care center... a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. This requirement is not met as evidenced by interviews and records review conducted during today’s complaint investigation. facility failed to notify Fresno Regional Office of the bookshelf falling on a child.

Mandated Reporter Training. On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training...and shall complete renewal mandated reporter training every two years following … the initial mandated reporter training. This requirement was not met as evidenced by records review. A staff file had an expired Child Abuse Mandated Reporter Training. This poses a potential risk to the health, safety and personal rights to children in care.
