Responsibility for Providing Care and Supervision - No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met as evidenced by: Based on records reviewed and interview there was a lack of supervision by a teacher, at the facility where an incident of inappropriate touching occurred due to lack of supervision. This poses an immediate risk to the health, safety, and personal rights of children in care.

The child care center shall be clean, safe, sanitary and in good repair at all times.This requirement was not met as evidenced by:LPA observed a bookshelf in the school age classroom that was wobbly. Additionally, staff admitted that the bookshelf had previously fell on top of a child. According to staff, child was not seriously injured, an incident report was completed, and the child's parent was notified.
