What the inspector wrote

In addition, through LPA observations and interviews, the allegation staff do not ensure that napping equipment is arranged in a safe manner is substantiated. On 07/01/2025 at 12:10PM to 12:15PM, LPA observed in P2 and P3 children napping in cots blocking the door exits. Therefore, the preponderance of evidence standard has been met, the above allegation is found to be SUBSTANTIATED. Exit interview conducted and report was reviewed with Director Jennifer Mays. Appeal rights were provided. Per Title 22, Division 12, Chapter 1 of the California Code of Regulations, the following deficiencies are being cited: (see 9099-D page). This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.

    (a) In child care centers providing meals to children, the following shall apply:(1) All food shall be safe and of the quality and in the quantity necessary to meet the needs of the children. Each meal shall include, at a minimum, the amount of food components as specified by Title 7, Code of Federal Regulations, Part 226.20, (Revised January 1, 1990) Requirements for Meals, for the age group served. All food shall be selected, stored, prepared and served in a safe and healthful manner. This requirement was not met as evidenced by:Based on LPAs interviews and records obtained, the licensee did not comply with the section cited above. On 06/23/2025, preschool classrooms were served undercooked hamburger patties which poses/posed a potential health, safety or personal rights risk to persons in care.

    (c) Equipment and activity areas shall be arranged so that there is no hazard from conflicting activities.This requirement was not met as evidenced by: Based on LPAs interviews, observations, records review and records obtained, the license did not comply with the section cited above. Licensee did NOT ensure the outdoor play area was free of hazard resulting in child 07 sustaining an injury which poses/posed a potential health, safety or personal rights risk to persons in care.
