What Happened
Annual inspection with deficiencies cited. Backyard fence hazard found with loose wooden plank providing access to public street. Child's file incomplete with missing form. Three staff members missing required immunization records and TB tests.
Observations
- Backyard fence perimeter: 1-foot-wide wooden plank loose and hanging providing 1-foot open access to back alley and main street - corrected during visit by licensee's son
- Child 03 file missing LIC9227 required form
- Licensee and 2 assistants missing immunization records for influenza, pertussis, and measles
- Licensee and 2 assistants missing TB test records
