What the inspector wrote

Licensing ProgramtAnalyst (LPA) Maddox met with Courtney Walsh, Director and Applicants, and Tammy Nguyen, Superintendent today for the purpose of conducting an announced pre-licensing inspection. Center is located within an apartment community and consist of 3 classrooms, office, Kitchen, bathrooms (including separate staff bathroom located in the front office area), and outdoor play space. Applicants are requesting licensure of 9 Infants; 16 Toddlers; 17 Preschools; The days and hours of operation will be: Mon through Friday from 8:00am -4:00 pm. INDOOR ACTIVITY SPACE : \302\267 The child care center was toured and found to be clean, safe, sanitary to ensure the safety and well-being of children, employees and visitors \302\267 Floors of all rooms have a surface that is safe and clean (carpet and tile). \302\267 A comfortable temperature for children shall always be maintained. \302\267 Furniture and equipment are maintained in good condition, free of sharp, lose or pointed parts. There are a variety of age-appropriate equipment, toys, and materials in good condition and in sufficient quantity to allow children present to fully participate in planned activities. \302\267 Tables and chairs were present to meet the needs of the children. \302\267 Drinking water is readily available - water bottle filling stations in the Toddler and PS classrooms, Infants will have filtered water available). \302\267 Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children were stored and inaccessible to children ( Stored in the Staff bathroom in a locked cabinet). Kitchen which is off limits to children. \302\267 There is a fully stocked first-aid kit(s) in locations accessible to staff but inaccessible to children. \302\267 The isolation area is located in the office for any child who becomes ill during the day. The front office area \302\267 LPA observed operable carbon monoxide detectors and smoke detectors (hard wired), fire extinguishers are located throughout the facility and fully charged. \302\267 The center has a working telephone \302\267 Sign-in and out procedure - Brightwheel and manually (if needed) \302\267 Staff shall conduct a wellness check to ensure that children with obvious symptoms of illness including, but not limited to, fever or vomiting, are not accepted. OUTDOOR Children have safe access to the play yard which is enclosed by a fence to protect children and to keep them in the outdoor activity area. Outside Drinking Water - LPA observed water fountains on the play yard. Page #2 Measurements taken were as follows: Infant Room (LPA explained the sleeping area is not counted in the measurements): 180/35 = 5 Toddler 525/35 = 15 Preschool 550/35 = 16 TOTAL INDOOR CAPACITY: 36 Outside Play Space: 4875-120 (shed) = 4755/75 = 63 (Staff will section off an area of the play yard for Infants and send in pictures) RESTROOMS: There are 2 separate children's bathrooms, each bathroom has 2 toilets and a Sanifount wash fountain (each has 2 spouts) that does no deliver hot water. All toilets and hand washing facilities were maintained in safe and sanitary operating condition. LPA observed soap , paper towels, and toilet paper available. SUPERVISION: Staff shall ensure no child is left without the supervision of a teacher at any time, Supervision shall include visual observation. INFANTS: Section 101430(3)(E) & 101439.1 Sleeping infant(s) shall be directly observed by sight and sound at all times. (A) This shall include when the infants are going to sleep, are sleeping, or are in the process of waking up. (B) Staff shall physically check on sleeping infant(s) every 15 minutes and document the following: \302\267 Labored breathing. Page #3 Signs of distress, which includes but is not limited to flushed skin color, increase in body temperature and restlessness. \302\267 Infants up to 12 months of age who are sleeping in a position other than on their back. \302\267 If the infant\342\200\231s Individual Infant Sleeping Plan [LIC 9227 (3/20)] does not have Section C completed, staff shall return the infant to their back for sleeping. If an infant falls asleep before being placed in a crib, staff shall move the infant to a crib as soon as possible. LPA will verify with manager regarding pack n plays for sleeping FOOD SERVICES Menu must be posted for parents\342\200\231 review. \302\267 Pesticides and other similar toxic substances were not stored in food storerooms, kitchen areas, food-preparation areas, or areas where kitchen equipment or utensils are stored. \302\267 Soaps, detergents, cleaning compounds or similar substances were stored in areas separate from food supplies. \302\267 Food-preparation and storage areas shall be kept clean and free of litter and rubbish; and measures shall be taken to keep all such areas free of rodents and other vermin. \302\267 Trashcans, including moveable bins, shall have a tight fitting cover that is kept on; shall be in good repair; and shall be leakproof and rodent-proof. \302\267 A refrigerator is available and shall be used to store any medication that requires refrigeration. RECORDS TO BE MAINTAINED AT THE FACILITY: LPA discussed form LIC311A Records to be maintained at the facility for child\342\200\231s records, personnel records, administrative records, and documents to be posted. Page 4 NAPPING \302\267 Cots used for napping were maintained in a safe condition. \302\267 Each mat shall be equipped with a sheet to cover the cot or mat and, depending on the weather, a sheet and/or blanket to cover the child. \302\267 LPA informed Applicant cots should be disinfected daily. \302\267 Bedding shall be individually stored so that each child's bedding is identifiable and no child's used bedding comes into contact with other bedding. \302\267 Napping equipment shall be arranged so that each child has access to a walkway without having to walk on or over the cots or mats of other children. Each child has an individual permanent or portable storage space for his/her clothing, personal belongings and/or bedding (cubbies). INFANT NAPPING \302\267 Each crib shall be occupied by only one infant at a time. \302\267 Bedding shall be changed daily \302\267 Cribs shall be free from all loose articles and objects, including blankets and pillows. \302\267 Pacifiers shall be allowed in the crib if the following provisions are in place: There shall not be anything attached to the pacifier. \302\267 Bumper pads shall not be used. \302\267 LPA observed Pack n plays arranged so as to provide a walkway and permit staff to reach each infant without having to step over or reach over any other infant. \302\267 LPA explained the following regarding Section 101439.1 Infant Care Center Sleeping Equipment : Mattresses shall be firm and covered with a fitted sheet that is appropriate to the mattress size, fits tightly on the mattress, and overlaps the underside of the mattress so it cannot be dislodged. \302\267 Prior to the infant's first day at the center, the infant care center director or assistant director shall complete a needs and services plan for the infant. Provided during inspection. Page #5 HEALTH RELATED SERVICES: Medications shall be kept in a safe place inaccessible to children. Kept in the office Applicant has implemented a written plan to record the administration of prescription and nonprescription medications and to inform the child's authorized representative daily when such medications have been given. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP) - Building was completed early 2025 POSTINGS: Applicant was informed of the required forms that shall be posted after licensure including the telephone number of the local health department and information on child passenger restraint systems pursuant to Health and Safety Code section 1596.95(g) and Vehicle Code sections 27360 and 27360.5 - Page #6 Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. To receive important licensed related information to licensed facilities, visit the CCLD Important Information website at https://www.cdss.ca.gov/inforesources/community-carelicensing/subscribe and select the Child Care option to receive email communication Applicant is reminded of the requirement to report and unusual incidents and/or injuries to the parent/guardian and Licensing within the time frame specified by the regulation and on the form LIC 624B. Each licensee shall have a disaster and mass casualty plan of action. The plan shall be in writing and shall be readily available. Fire Clearance been received for the requested capacity of 46, however, LPA's measurements were as follows: 5 Infants; 15 Toddlers; and 16 Preschool. Needed Prior to Licensure: LPA reviewd the NOIA letter and explained what additional documentation is missing and/or needed. No physical plant corrections needed. There are large trees that surround the play yard, however, during this inspection, the trees did not provide any shade, additional shade should be available for the afternoons (Director states trees provide shade in the mornings). Shared Play yard Waiver needed - LPA will email sample request. An exit interview was conducted with the above items discussed and a copy of this report was provided to the Director. Final license determination will be made upon review of the Licensing Program Manager.
