At 12:15pm on 3/20/2025, Licensing Program Analyst (LPA) Matthew Gallo met with applicant Shelby Clark for the purpose of a change of location prelicensing inspection. Applicant has requested a Small Family Childcare Home License to serve up to 8 children, and was previously licensed at facility #

.
The facility is a one story, 3 bedroom 2 bathroom home consisting of a Living Room, Kitchen, Dining Room, Hall Bathroom, Master Bedroom and Master Bathroom, Bedroom 1 (first room to the right of the hall) and Bedroom 2 (room at the end of the hall). The property features a detached garage.
Off limit areas will include: Kitchen, Master Bedroom/Bathroom, Bedroom 2, and Garage.
Applicant understands that children must never enter these areas. Applicant also understands that any unfenced areas of the property require 100% supervision.
Applicant intends to operate 7:30am-5:30pm, Monday-Friday, year-round. Applicant understands they must be present at the facility for 80% of operating hours per day. Applicant understands that their own family members under 10 years old count toward the ratio and capacity of daycare children.
All individuals subject to criminal background review have obtained a criminal record clearance. Applicant was
reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

LPA and applicant toured the entire home and property. All toxic and hazardous items are inaccessible to children and applicant understands that any cabinet containing such items and is reachable by children must be made inaccessible. Applicant acknowledges that any poisons must be made inaccessible by key or combination lock. LPA observed a 2A10BC fire extinguisher along with functioning smoke and carbon monoxide detectors. Applicant stated that there are no firearms in the facility, and LPA did not bodies of water on the property. Wood burning stove is barricaded to prevent access by children. There are no stairs in the home.
Applicant provided proof of control of property in the form of a lease agreement. Applicant understands that until a liability insurance coverage in the amount of $300,000 is provided, the affidavit LIC282 form will be used. Because the applicant rents/leases the home, proof of landlord notification is required. On 3/20/2025, the California Attorney General - Megan\342\200\231s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.
As of today, March 20, 2025, this facility is approved for a Small Family Child Care Home license for a capacity of 6 children with no more than 3 infants, or 4 infants only; or up to 8 children with no more than 2 infants, with 1 child in Transitional Kindergarten or above and 1 child at least age 6. Infants are children under the age of 2.
