What Happened
Case management incident investigation found that child C1 walked away from supervised playground and was left unattended on a ramp to Classroom #2 for approximately 2-4 minutes while staff assisted a parent with sign-out procedure.
Observations
- Child C1 walked away from playground and was found sitting alone on ramp by parent
- Child left unattended for approximately 2-4 minutes while staff assisted parent with sign-out
- Staff #1 was looking at sign-in/out app on parent's phone when child wandered away

(1)No child(ren) shall be left without the supervision of a teacher at any time....Supervision shall include visual observation.This requirement was not met as evidenced by: Based on LPA's interviews with 4 staff and observation of the physical plant it was determined C1 wandered away from the lower play yard under the supervision of S1 and was found alone on the ramp to classroom #2 by P1. C1 was left unattended for approximately 2-4 minutes. This imposes an immediate health and safety risk to children in care.
