C1 did not have care and supervision for approximately 5 minutes. This is an immediate risk to the health, safety risk to chidlren in care.

The facility could not provide a copy of the completed lead testing and LPA did not observe lead testing completed on the waterboard website. (1)No child(ren) shall be left without the supervision of a teacher at any time....Supervision shall include visual observation.This requirement was not met as evidenced by: Based on LPA's interviews with 4 staff and observation of the physical plant it was determined C1 wandered away from the lower play yard under the supervision of S1 and was found alone on the ramp to classroom #2 by P1. C1 was left unattended for approximately 2-4 minutes. This imposes an immediate health and safety risk to children in care.
