    Based on observation and interview, the licensee did not comply with the section cited above. During indoor activity space inspection, LPA observed 4 rugs located in classrooms 1,2, and 3 dirty. Director stated the carpet's were last cleaned 6 months ago. This poses/posed a potential health, safety or personal rights risk to persons in care.
