What the inspector wrote

On 8/21/2025 an unannounced case management inspection was conducted on this date by Licensing Program Analyst (LPA) Navar in response to a self-reported incident dated 8/11/2025. LPA met with Director Aimee, Valentin and was led on a tour by Elizabeth, Cuatecontzi. The census was taken and there were 38 preschool children and 4 staff present at the time of inspection. Some children were napping, and some children had just woken up. A review of adult records indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. The Department received an incident report on 08/11/2025 stating that on 08/11/25 at 10:30AM S1 had observed C1 had been found in the classroom alone behind the classroom front door at 10:31 by S2. During the investigation LPA interviewed 1 (one) staff (S3) who stated as classroom was transitioning to playground at 10:30AM, S2 had completed face to name recognition during daily safety check and escorted the children out of the classroom to the playground. At 10:31AM S2 opened the classroom front door and found C1 alone behind it. S3 stated a meeting with the staff regarding proper usage of face to name and purposeful transitions has been completed and managers support classrooms when transitioning children to and from the playgrounds. During today’s visit 8/11/2025 LPA reminded S3 of Title 22 Regulation 101229 Responsibility for Providing Care and Supervision (a) The licensee shall provide care and supervision… (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. Continue to page 2. Page 2 Based on the information gathered from the interviews with staff and records review of the Unusual Incident Report dated 8/11/2025. The facility was not in compliance of the California Code of Regulations, Title 22, Division 12 101229(a)(1) Responsibility for Providing Care and Supervision. It was determined that 1 Type A citation will be cited during today’s visit. See the attached LIC 809D. LPA Navar informed director Aimee, Valentin that this report dated 08/21/2025 for Type A citation shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care. Also, LPA Navar informed the director Aimee,Valentin to provide a copy of this licensing report dated 08/01/2025 Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted and report was reviewed with the Director Aimee, Valentin. A notice of site visit was given and must remain posted for 30 days. Failure to post will result in civil penalties of $100. End of Report.
